Osteoporosis: Risk, Diagnosis, and Fracture Prevention

OVERVIEW
Osteoporosis is a progressive skeletal disease characterized by low bone mass and microarchitectural deterioration of bone tissue that is associated with significant fragility fractures and morbidity. By one estimate, 10 million people in the United States have been diagnosed with osteoporosis, and an additional 34 million with low bone mass are at an increased risk to develop the disease.
Diagnosis of osteoporosis is largely based on the measurement of bone mineral density (BMD). Testing for BMD and the use of T-scores permits the diagnosis to be made and treatment to be initiated in individuals who have not sustained a fragility fracture, but who may be at risk of future fracture. Nonetheless, it is now evident from the National Osteoporosis Risk Assessment study that one-half of osteoporotic fractures occur in women whose T-scores are above -2.5. These limitations have led to the development of the FRAXTM tool, which integrates other independent risk factors to provide an algorithm to assess fracture risk levels. Most of the available therapeutic agents for the prevention and treatment of osteoporosis are antiresorptive. However, all patients benefit from nonpharmacological lifestyle interventions, such as correcting calcium and vitamin D deficiency, exercising, smoking cessation, and moderating alcohol intake.
Using case vignettes, this activity will discuss the use of BMD testing and FRAXTM in the diagnosis of osteoporosis, identify treatment strategies for patients with acute fractures, discuss risk factors and prevention strategies for nonvertebral fractures, and outline appropriate and effective treatment options.
